Ban requests?

Just wondering what the deal is in relation to ban requests? Sorry if I'm missing something obvious. I've been getting trolled by this same guy consistently now every time I see him-- always evading the admins. Starting to piss me off. I remember there being a forum or something before for ban requests with a template you had to fill out but I can't seem to find anything.
